Minimally Invasive Spine Surgery

Minimally Invasive Spine Surgery (MISS) is a progressive spine relieving method which allows performing back surgery without long incision as it traditionally done with open spine surgery. Actually, there are a few types of surgery referred as minimally invasive that have the save purposes but are different in performance. In Clinic of Braunschweig we used different methods choosing a proper procedure based on individual situation of patient.

Types of Minimally Invasive Spine Surgery

There are two main ways to perform minimally invasive spine surgery:

  • Mini-open. It is open surgery but with access via smaller incision.
  • Percutaneous. This type of surgery is made through the skin.

The last type if MISS can be subdivided to:

  • Tubular
  • Endoscopic

Tubular Spine Surgery

Tubular technique is involved a tubular retractor which is a special tube-like tool to split the muscles and allow access inside the spine. To open wider area a few expanding tubes can be inserted and a surgeon performs manipulations through a pipe hole.

Tubular method is widely used in spine neurosurgery. Among the types of spine operations carried out using retractor you could name microdiscectomy, thoracic spine decompression, treatment for cervical foraminal stenosis, tumor removal, laminotomy, spinal instrumentation (fusion) and others.

Endoscopic Spine Surgery

Endoscopic spine procedure is a kind of percutaneous surgery using an optical telescopic tube named ‘endoscope’. An endoscope is inserted through a small incision to give the surgeon visualization of performance field in digital form. Using a camera allows to precisely see the nerve roots and other zones that are not visible by normal vision. Another incision is used for introducing of miniature surgical tools. Endoscopic minimally invasive surgery for spine is not as common as above described mini-open and tubular procedures but some surgeons find them beneficial for specific spine operations as discectomy. Endoscopic discectomy is a technique specially developed for degenerated interverberal discs treatment.

What Conditions Are Treated with MISS?

Microdiscectomy

Miscrodiscectomy is one of the most demanded minimally invasive procedures for spinal disks treatment. Its main goal is decompression of the nerve roots and relieving a pain. Compression of the spinal nerves or spine cord happens due to degenerative disk disease or spine disks herniation. During the surgery small portions of damaged or bulged disk tissue is removed to give more space to the nerves.

Minimally Invasive Cervical Spine Surgery

Minimally invasive cervical spine surgery is a treatment for spine diseases of the neck and upper back. It is not so demanded comparing to lumbar spine surgery but also recommended when the other ways of pain relieving in cervical spine were not effective. Classical approach to the cervical surgery is posterior invasion but sometimes anterior surgery for neck problems can be decided. Posterior approach is often used for cervical discectomy when treatment of protruded disks is needed. It is considered good for cervical herniations. Anterior approach have advantage against posterior cervical surgery is that it gives easier access and it is possible to perform not only discectomy but also fusion procedure which is reducing vertebral flexibility to limit pain causing factors.

Laser Spine Surgery

Laser Spine surgery as non-invasive procedure for herniated disks is also used alongside with minimally invasive techniques. It is considered especially effective for big softer herniations allowing removing tissues with focused laser beam. As laser is not beneficial in removing bones, it is often used in combination with other types of minimally invasive surgery.
